Should you decide to rob Arvo, you'll have painkillers in your inventory. Naturally, the game tries to convince you to give them to Rebecca, but you can choose not to. If you don't give them to Rebecca, you will keep them in your inventory going into episode 5.
When you are talking with Luke, you'll be given the option to give him the painkillers. If you choose this option... you give him the painkillers!
You can also choose not to give them to Luke either, and that results in you carrying the painkillers the rest of the game with no further opportunities to use them, and makes the choice of robbing Arvo absolutely pointless.
